(June 10, 2008 - Ann Arbor, Mich.) -- All individuals who will be competing on one of the 15 varsity sports at Concordia University-Ann Arbor will need to have the appropriate documentation completed and turned into the Cardinal Athletic Office prior to arriving in the fall. The forms can be found by clicking on the Sports Medicine link on the left-hand side panel of the Concordia Athletic website. Each athlete, whether returning or new, will have two documents they must complete. The documents are labeled easily: all, new and returning.
